Fistful of Zombies 2.0 delivered the biggest gameplay addition, adding night settings, personalized zombie skins, and new mechanics to the zombie mode. The same update introduced Knife Fight Club, while Halloween and Christmas brought new maps, mods, collections, and returning modes.

Horse Riding was the biggest addition, bringing mounted movement, stronger attacks, ramming, and horse-exclusive maps to Fistful of Frags. The game also added the Slide and Jump Master perks, the FragMaster progression system, and the optional Retextured DLC.
- Mounted combat: Horse Riding let players aim and reload without a movement penalty, ram opponents, and use a control scheme based on standard first-person movement. New horse-exclusive maps supported the feature.
- Retextured DLC and map content: Fistful of Frags: Retextured DLC restored the stylized texture set with additional textures and improvements as an optional download. The Tortuga map gained more connections, escape routes, and revised gameplay, while other maps received safer spawning, improved player flow, secret rooms, and stationary Gatling guns.

Halloween 2018 Update delivered the limited-time Ghost Town mode, two new levels, supernatural weapons, and roaming ghosts. The December gameplay overhaul then added the Hammerless Pocket Revolver, the Tracking Skill, and broad weapon, movement, and scoring changes.

The Yellowboy entered the loadout menu with 30% reduced damage, while the Schofield and Smith Carbine left the stock-item pool.

Teamplay arrived as the year’s biggest structural addition, bringing team-versus-team objectives, purchasable weapons, team balancing, and zone capture. It was followed by Break Bad, Ghost Town, and Elimination, giving Fistful of Frags several distinct ways to play beyond standard Shootout.
- Teamplay expanded into a full mode family: Teamplay launched with tp_riobravo, then added tp_loothill, tp_eliminator, tp_snowy, tp_forest, tp_yosemite, and tp_abandoned. Zone Capture, the hybrid zone-capture/elimination ruleset, and the asymmetric Escape mode added objective-focused variations.
- Break Bad and Gang Standoff added new match structures: Break Bad put players on the map unarmed, made weapons purchasable through cash earned in combat, and penalized attacks on unarmed players. Gang Standoff delivered four-team, no-respawn arena rounds, while Top of the West added server-based rankings and statistics for competitive Teamplay.
- Ghost Town delivered the major seasonal event: Ghost Town introduced last-man-standing rounds with playable ghosts, ghostly weapons, and the Golden Skull objective. The Halloween release also added gt_creepycreek, gt_desperados, the Ghost Buster and Skull Collector achievements, and a ghost-speed and melee-damage tuning pass.
